You don't need cards specifically. You are supposed to have a "flight test plan". In general terms that simply means that you have read and plan to follow the guidance found in FAA advisory circular AC 90-89A, which is titled "Amateur-Built Aircraft and Ultralight Flight Testing Handbook". There are some generic flight-test cards included in that AC, but I'm sure you can find other examples by simply searching the internet. But as long as you show knowledge of the flight testing AC, you have covered the requirement.
