FAA program letter template - fill in

Hey all,
I could not find a fill-in template for the program letter, so I created one. Check it out. Feedback or alternate versions are welcome.

It would not let me upload a .xlsx file. I saved it in .xls format and I created it in .ODS format.

I ran the program letter past my DAR, and he had this comment:
ALSO - in your Program Letter - include a line or so stating: "Aerobatic Flight to be conducted.". Your Weight and Balance data indicates aerobatic flight. The wording in the Program Letter gives credibility when i check that block for your Operating Limitations. This does not mean you have to do any aerobatic flight - but if the urge ever strikes - you have the Limitations for it.
